Health System Cloud Migration to HIPAA-Compliant AWS with Real-Time FHIR Exchange

A multi-site hospital network operating aging on-premises infrastructure across facilities acquired separately over years could not share patient data in real time, could not meet evolving HIPAA technical safeguards uniformly, and faced growing pressure from payers to support FHIR-based data exchange. The Challenge

Each hospital site had evolved its infrastructure independently — different EHR configurations, different compliance postures, and no shared data layer. Clinical staff transferring patients between facilities relied on manual record reconciliation, fax-based sharing, and phone calls to piece together prior history. Delays were common; gaps in patient records were routine.

Regulatory pressure compounded the operational problem. CMS interoperability mandates required FHIR-based patient data access. HIPAA technical safeguards needed consistent enforcement across every site. The aging on-premises environment could satisfy neither — and a site-by-site compliance effort would have taken years.

Our Approach

Sequoia migrated the network incrementally — one site at a time, with automated rollback triggers — while building the shared FHIR data layer that made the unified environment more valuable than the sum of its parts:

• HIPAA-compliant AWS architecture — HIPAA-eligible AWS environment designed across all sites — VPC isolation, encryption at rest and in transit, CloudTrail audit logging, and AWS Config continuous compliance monitoring. Every service choice validated against HIPAA technical safeguards before deployment.
• HL7 FHIR R4 integration layer — Real-time FHIR R4 integration layer built on AWS HealthLake and custom API gateways — enabling bidirectional clinical data exchange between sites and downstream systems including EHRs, lab information systems, and payer portals.
• Zero-downtime migration — Legacy clinical databases and application servers migrated incrementally using a blue-green strategy. Automated rollback triggers fire if any health check fails during cutover — clinical operations are never at risk.
• HL7 v2 to FHIR translation — Legacy HL7 v2 messages translated to FHIR R4 in real time via a managed translation service. Existing clinical systems continue operating unchanged — no rip-and-replace of EHR or clinical applications required.

The Outcome

Clinical staff across all sites now access up-to-date patient records regardless of where care was originally delivered. The manual inter-site coordination that previously required phone calls and faxes has been replaced by real-time FHIR data exchange — eliminating a documented patient safety risk and reducing administrative overhead across every department.
